Program Forms

Forms for all SCIC programs are listed in alphabetical order by name of the form.

The small icons in the right corners also provide examples and instructions for completing the form.

For Crop Insurance: Access program forms, endorsements and the Contract of Insurance.

For AgriStability: Individual producers (sole proprietors) must submit their income and expense (tax) information on the T1163 form to the Canada Revenue Agency, who shares this information with SCIC. Corporations and Co-operatives must submit all AgriStability forms to SCIC. An individual’s supplemental information (inventories, receivables, payables, deferrals and purchased inputs) must be submitted directly to SCIC.

Corporations/Co-operatives/Other Entities

2024 Program Year

Trusts, Corporations and Co-operatives, Communal Organizations, Status Indians/Band Farms and Limited Liability Partnerships participating in AgriStability are required to submit financial and supplemental information to SCIC using this form. This information is used for both the AgriStability and AgriInvest programs and needs to be sent directly to SCIC.

Greenhouse and Nurseries Productive Capacity

Interim Benefit Application

2025 Program Year

The AgriStability interim benefit option allows you to access funds prior to the completion of your fiscal period in the program year. The interim benefit is based on your estimated margin decline in the program year, relative your estimated margin decline in your reference period.

Livestock Inventory Calculation Worksheet

To assist in calculating ending inventory amounts necessary for completing your AgriStability forms, this worksheet is for your own reference and is not required to process your application. Transfer the amounts from the lettered columns to the corresponding columns on your AgriStability forms.

Participant Initial Declaration Form

All AgriStability participants must sign the declaration to confirm they understand the terms set out by SCIC. The declaration outlines the roles and responsibilities of a participant in the AgriStability program. This is a one-time requirement that allows SCIC access to your AgriStability program information. Failure to submit this declaration may result in a loss or delay of your AgriStability benefits. The Participant Initial Declaration is mailed to participants.

Supplemental Information

2024 Program Year

AgriStability individual participants (sole proprietors) are required to submit supplementary information directly to SCIC. This includes crop and livestock inventories, purchased inputs, deferrals, receivables and payables information.
