Program Forms

Program forms for all SCIC programs are available online, including document examples and instructions to help you complete each form. All forms are listed in alphabetical order by name of the form.

For Crop Insurance customers: Access program forms, endorsements and the Contract of Insurance.

For AgriStability participants: Individual producers (sole proprietors) must submit their income and expense (tax) information on the T1163 form to the Canada Revenue Agency, who will share this information with SCIC. Corporations and Co-operatives must submit all AgriStability Program forms to SCIC. An individual’s supplemental information (inventories, receivables, payables, deferrals and purchased inputs) must be submitted directly to SCIC.

2022 Program Year

Trusts, Corporations and Co-operatives, Communal Organizations, Status Indians/Band Farms and Limited Liability Partnerships participating in AgriStability are required to submit financial and supplemental information to SCIC using this form. This information is used for both the AgriStability and AgriInvest programs and needs to be sent directly to SCIC.

Supplemental Information

2022 Program Year

AgriStability Program Individual participants (sole proprietors) are required to submit supplementary information directly to SCIC. This includes crop and livestock inventories, purchased inputs, deferrals, receivables and payables information.

Additional Information Amendment

This form can be used to provide additional information to support the data on your AgriStability program forms or to justify an amendment request.

Amendment Request

This form can be used to request an amendment for your AgriStability program forms. You can request an amendment to information used in calculating your program benefits for a program year by submitting the request to SCIC up to 18 months from the date of the original Calculation of Benefits.

Appeal Submission Form

If you feel the AgriStability program rules were not correctly applied in the processing of your application, you may request a review of the application by SCIC. If SCIC cannot resolve the concerns, the request may be referred to an appeal panel. Appeals must be submitted in writing to SCIC within 90 days of the date you were notified of the decision which is being appealed.

Cancel Participation

Participants wishing to cancel participation in AgriStability must complete and return this form to SCIC by the applicable deadline. If you are changing to another type of farming entity (corporation, trust, etc.) you must complete a Change Business Type form.

Change Business Type Form

Farming operations that have gone through a change in their business structure need to report that change as part of filing for AgriStability. For example, if a farming operation has transitioned from a sole proprietor to a partnership or corporation, simply complete the Change in Business Type Form. This form outlines the change and ensures your historical farm information is transferred to the new business structure.

Change Contact Person Form

Participants who wish to change/update their contact person can complete and return this form to SCIC. In naming a contact person, you authorize that person to receive, provide or make changes to information on your behalf concerning the AgriStability program.

Change Participant Information Form

AgriStability participants who wish to change/update their personal information, such as name or address, can complete and return this form to SCIC. If you are changing your name, you must supply supporting documentation such as a marriage certificate or other proof of the name change.
